Appointments

• We welcome all new patients

• We try to do our best to accommodate our new patients within 7-10 days

• In cases of emergencies, appointments are made within 24 hours

• Cancellations should be made with at least 24-hour notice

Please, bring the following to your appointment:

• Insurance card

• Physician referral forms, if required by Insurance

• List of your medications, if you take any

• Recent relevant medical records

Insurance and payment information

• We accept Medicare, MassHealth, major HMO and private Insurances

• You are responsible for all charges not covered by your Insurance at the time of your appointment

• Your Insurance Company may require a co-payment at the time of your appointment. We accept MasterCard, Visa, personal checks and cash
